About Physician Health Partners
Physician Health Partners (PHP) and its wholly-owned subsidiary, Correctional Health Partners (CHP), is located in the heart of downtown Denver and focuses on improving the healthcare delivery for thousands of Coloradans. Their goal is to improve the health outcomes and experience for their patients, while managing the associated costs. The company was founded in 1996 by primary care physicians and is still governed by physicians. They work alongside over 300 primary care physicians and hundreds of specialists across the Denver Metro area to improve the quality and coordination of healthcare for thousands of patients.
In 2012, PHP was named one of 32 Pioneer Accountable Care Organizations in the US chosen by to help improve Medicare for thousands of Denver-area patients. They are also part of the collaborative Colorado Community Health Alliance and provide health coordination for thousands of Medicaid members in the Denver-area. CHP works with Colorado's and Oregon's Department of Corrections, and other corrections systems providing necessary care to offenders, while lowering or containing the costs.
